If your name is different from the name on your current driver license, you will need to provide legal proof of the name change with a certified state marriage certificate, divorce decree, adoption papers, citizenship papers, guardianship papers, or a court document indicating the name change. You will then need to follow the requirements for either a Utah renewal, duplicate or new to the State of Utah providing proof of birth, full social security number and residence.
