Notice: Change to Medical Card Requirements

ALERT **On May 21, 2014, Utah CDL drivers needing a DOT card will be required to have their medical examinations performed by a Certified Medical Examiner listed on the Federal Motor Carrier Safety Administration's National Registry.  https://nationalregistry.fmcsa.dot.gov/NRPublicUI/home.seam

DOT card issued before May 21, 2014 will be honored until their expiration date.  Cards issued on or after that date will not be accepted by the Division and will need to be issued by a provider on the National Registry.


The Federal Motor Carriers Safety Administration has issued rules that require Commercial Driver License holders to submit a current Department of Transportation (DOT) medical card and medically certify what type of driving you are conducting to the Utah Driver License Division.

Beginning January 30, 2012, CDL holders will be required to submit a valid DOT medical card and medical self driving certification for original, renewal, upgrade, duplicate and state to state transfer license applications.  After implementation all CDL drivers will be required to submit a new DOT card and medically certify when the previous DOT card expires.  All CDL drivers must comply by January 30, 2014.

The Division will be sending letters to all CDL holders notifying them of the changes.  The Medical Self Driving Certification will be located on the back of that letter and should be returned with the DOT card.

Drivers may begin sending in the DOT Card and Medical Self Driving Certification to the information below.  Please DO NOT send the original DOT Cards or Long Forms because they will not be returned and must be carried with you.

Or you may fax a copy of your DOT card or long form and the enclosed medical certification to 801-957-8633.  Please make sure you fax the certification and not the letter.

Here is what you can do to ensure the process is complete.

Make Sure your DOT Card or Long Form has the:

  • Doctor's First and Last Name;
  • Doctor's Phone Number;
  • Doctor's License or Examiner Number;
  • Doctor's State Where licensed;
  • The DOT exam date or Issue date; and
  • DOT Card expiration date

Make Sure your Medical Self Certification:

  • Is checked in one of the 4 categories;
  • Has your printed name and your signature;
  • Your driver license number; and
  • And your date of birth

If any of the information listed above is incomplete your medical certification cannot be completed and will be returned to you for the additional information.
