Administrative Office Services:

  • Original, Renewal and Duplicate licenses are not done at this office, please visit one of our licensing offices (see list of all offices to the right)
  • Questions concerning DLD operations call: Customer Service 801.965.4437 or toll free at 888.353.4224
  • You can purchase your driver history record (MVR - Motor Vehicle Report) here
  • You can pay to reinstate your driver license here
  • You can pick up your surrendered driver license or ID card
  • Payment by Cash, Check, Money Order, Debit Card and Credit Cards are accepted at this location for reinstatements of license
